Do I sign up for Cobra with my employer or Blue Cross?

My coverage runs out end of month.  How do I sign up for Cobra until I am covered by new job? Directly you or my employer?  If I have a claim during lapse of time we are doing paperwork am I covered? Is it retroactive to the date of the termination of my plan through my job?

    Husband recently lost his health coverage due to losing his job.  He is in need of going to the doctor as he has a possible blood clot and Factor 5 disorder.  His employer hasn't sent any COBRA information so not sure where to go from here.  We can still log in and see his claims but can't find anyway to continue his coverage.
