I have had several claims denied. The stated reason for denial is that my plan does not cover non-emergency treatment by an "out of area" provider. My plan is a Medicare Supplement Plan (not Medicare Select or Medicare Advantage) and I have been told that BCBS does not have the legal authority under federal Medicare regulations to deny a Medigap claim based on "non-emergency" status or because the provider is "out of area". If this is correct I would like to have these claims reviewed and adjusted without me having to go through the formal appeals process. Please advise me if this can be done and what additional information is needed from me to initiate such a review and adjustment.
